Verify Patient Name/Address / Phone Each Visit
Each visit, ask the patients if any of their demographic information has changed. If so, have them complete a new Patient Information Sheet for name/address/phone number changes. Keep the original & forward a copy to, or include the new info with, documentation sent to your biller.
Verify Patient Insurance/Copy Front & Back of Cards
Always ask if any insurance information has changed since the last visit. If so, be sure to have them complete an Insurance Information Sheet & photocopy the front and back of the insurance cards. Keep copies for office records and forward copies to your biller.
Physicians-Have Patients Sign an Authorized Release of Information Form
Have the patient sign new forms the first visit of each new year. Keep this on file in your office.
Waiver of Liability/Advance Beneficiary Notice Form
For Medicare patients, be sure to have them sign this form for charges that may not be covered BEFORE the services are rendered if the patient agrees to pay for services Medicare may not cover. Otherwise, the charges must be written off.
Notice of Privacy Practices
As of April 14, 2003, it is a HIPAA requirement to inform patients of your policy regarding the privacy and access of their protected health information (PHI). You'll need to post this in your office as well as give these notices to patients to read. Make sure it states who your Privacy Officer contact is should they have questions or wish to file a complaint.
Medicare One-Time Authorization
Make sure patients sign a form authorizing the provider to receive payment for services rendered and allowing release of medical information. For DME, a new form must be signed for each new item.
DME Same/Similar Equipment
DME suppliers please be sure to ask the patients if they have ever rented or purchased same or similar equipment in the past. If so, be sure to have the patient sign an Advance Beneficiary Notice and explain that Medicare may not pay the claim without information submitted regarding the previous equipment.
